Hospitalization Procedure

  • Please visit the reception counter (No.9) on the 4th floor.
  • Please bring the following materials on the day of your admission.
    1)Application Form for Admission ・ Certificate of Guarantee of Patient’s Identity and Medical Expense Payment.
      (Please use a prescribed form of the hospital.)
    2)Health Insurance Card
    3)If you have any medical care certificates, please present them with your health insurance card.
    Your expenses are covered by public funds.

     Without the certificates, you must pay all the medical expenses by yourself.
  • Please visit the overtime reception counter after-hours.
  • We have staterooms (pay rooms). Please ask the reception counter (No.9) on the 4th floor. (They are limited and are subject to availability.)

    〈Stateroom〉
    If you stay overnight, you are charged the cost of 2 nights hospitalization.
     ※You can watch TV for free for stateroom A and room type A.
     ※You can use free internet for all staterooms.

Preparation for Hospitalization

  • Please bring the following items upon admission:
    ID Card Toiletries Slippers
    Health Insurance Card Chopsticks Trash can
    Teacup Underwear Writing materials
    Kleenex    
  • Minimum necessary personal items. Please do not bring unnecessary cash or valuables.When you have cash, please use a security box in each patient’s room.(Be sure to lock it by yourself and to carry a key.)
  • You can use your own gown or patient gowns are available for 50 yen per day. We change it twice a week.
  • Please switch your cell phone to silent mode while in this hospital. You are free to use your phone in the cafeteria, waiting room, telephone boxes and all hall ways except those in wards.

Being in the Hospital

  • Meals
    • Meal time: Breakfast 8:00 a.m.  Lunch 12:00 p.m.  Dinner 6:00 p.m.~  Please do not bring any foods and drinks to avoid the risk of food poisoning.
    • Please bring your own chopsticks (spoon, fork), teacup.
    • You will have the option for your meal from the general menu.
  • Mail Box
    • A mail box is located by the entrance.
  • TV  
    • You can watch TV with TV card. There are vending machines located on each floor.
  • Laundry  
    • Laundromats are located on each ward.
  • Leaving or Staying out
    • Please ask your doctor or nurse if you wish to leave the hospital for a brief amount of time or spend the night outside of the hospital.
  • Please consult your nurse if you wish to take a walk or exercise.
  • Emergency
    • In case of an emergency, please follow all staff instructions. (The refuge is Ninomaru park.)
  • Facilities  
    • A Lawson, restaurant, and ATM are located on the 4th floor.

  • The Others  
    • Please do not walk in public areas wearing only patient gown.
    • Please do not park your car during your hospitalization.
    • Lights out at 10:00 p.m.
    • Smoking is not allowed anywhere in the hospital.
    • Drinking alcohol is strictly prohibited.
    • Please inform the reception counter (No.5) if your Health Insurance Card has been changed or expired.
    • Please ask a ward head nurse if you need your medical certificate or any other certificates.
    • We ask all inpatients to wear a wristband during their hospitalization.
    • You are free to ask our staffs any questions.
    • We cannot accept any gifts from patients.

For Visitors

  • Visiting Patients
    • Visiting Hours : Weekdays 2:00 p.m. – 8:00 p.m.  Sat, Sun, Holidays 10:00 a.m. – 8:00 p.m.
    • Please ask permission from a staff station to visit. As a general rule, visiting hours are within 30 minutes.
    • Visitors may be requested to refrain from visiting depending on the patient’s or other patient’s condition.
    • Please refrain from eating or drinking in the patient’s room.
    • Children are not allowed to visit patients to disturb their rest cure.
  • Attendant  
    • It is usually not necessary to have an attendant. However, if a family member needs to stay with a patient, doctor will allow your request.
  • Other Notices  
    • Please do not call a patient unless it is an emergency.
    • The address of the mail, please write the name of the ward.
  • Parking  
    • Parking is available in this hospital for visitors.

Hospitalization Expenses

  • Payment
     <Discharge>
    • We will infrm you the amount of money due the day before your discharge. Please pay the medical bill at the cashier (No.5) on the 4th floor. Credit cards and cash cards are also accepted for payments.
    • Please present your health insurance card and other public funds on the day of your discharge. Please ask the staff about any documents that you may need before being discharged.
