First Visit
Fill out a consultation form at the reception desk on the 4th floor (main floor) and present the form and your health insurance card together to Widow 2. If you have a referral form, please present it at the window.
Registration starts at 8:15.
Revisiting
Check-in first at the Automatic Check-in Machine and take a registration slip and your clinic ID card. Please keep them in a file and take it with you.

We basically follow an appointment system; therefore, please make an appointment for a future visit with a doctor-in-charge before you leave.
Present your clinic ID card and Health Insurance Card to Window 3 (second visit).
Clinic ID Card
A clinic ID card is commonly used for all departments, so please keep the card in a safe place. Be sure not to lose it. If you lose your card, please go to Window 3 for a reissue.
Health Insurance Card
On your first visit every month, you will need to present your health insurance card, so please bring it with you. Without the card, charges may not be covered under the insurance plan.

You are called by a number on a registration slip to protect your privacy.
Check-Out and Payment
After a consultation or treatment, please go to Window 4 and present your file, as well as your treatment schedule, out-hospital prescription and so on. You will be called by the number on your registration slip from Window 5, when the calculations are finished.
Payment by ATM or credit card is accepted, as well as by cash.
ATM cards are available only when co-payment is over 1250 yen and can be used from 8:00 a.m. to 9:00 p.m. on weekdays and from 9:00 a.m. to 8:00 p.m. on weekends.
Out-hospital Prescription
Our hospital basically keeps an out-hospital prescription system. You can get a prescription after payment at Window 5. If you need further information about the most convenient pharmacy in your area, please go to “Out-hospital prescription information desk”.
Medical Documents
If you need copies of medical documents -- such as a medical certificate or physician’s statement, please visit Window 6. The charges for those documents are not covered under the insurance plan. Please ask Window 6 for further questions.
